RELEASE/WAIVER
The undersigned represents and warrants that (s)he is a licensed skin care professional in the state in which (s)he practices.
The undersigned further understands that chemical peeling solutions with concentrations of 20% or greater, including but not limited to Glycolic Acid, Lactic Acid, Salicylic Acid, TCA (Trichloroacetic Acid), and combination peels, are professional-grade products intended for use only by trained and licensed professionals.
The undersigned acknowledges that improper use of professional-grade chemical peeling solutions can result in serious skin damage, scarring, burns, hyperpigmentation, and other adverse effects.
The undersigned agrees to:
- Use all chemical peeling products strictly in accordance with manufacturer guidelines and professional training
- Perform appropriate patch tests before full application on clients
- Maintain proper client consultation records and informed consent documentation
- Not resell professional-grade products to unlicensed individuals or the general public
- Accept full responsibility for the proper and safe use of all products purchased
By signing below, the undersigned releases and holds harmless A Natural Difference, its owners, officers, employees, and agents from any and all claims, damages, losses, costs, and expenses arising from the use or misuse of professional-grade chemical peeling products purchased from A Natural Difference.
This waiver shall remain in effect for all future purchases of professional-grade chemical peeling products unless revoked in writing.
